Every organization has an official version of how work gets done.  These are often documented in the SOPs, the system configurations, the org charts. However, every organization has the actual version: the exceptions, the manual overrides, the individual spreadsheets

That means walking the floor, sitting with planners and buyers, and watching transactions happen in real time rather than relying on the process map. The goal isn't to catch anyone doing something wrong, but rather  to find where the documented process quietly stopped being true, because that gap is usually where the real cost is hiding.

Surface the spreadsheets, shadow systems, and manual processes that have been created due to eroding system trust

When people lose confidence in a system's information, they don't stop working. Instead, they build around it. The shadow systems can be found in a spreadsheet here, a side calculation there, a second inventory count nobody officially tracks. Individually these look like small fixes. Collectively, they're a sign that the organization has stopped trusting the system of record, and every one of them is a place where data, visibility, and accountability quietly leak out.

We inventory these workarounds deliberately, not to eliminate them, but to understand exactly what job each one is doing. Most workarounds exist because they're solving a real problem the system isn't solving. You can't fix the system until you know where or why it is failing.
